The Primary Care Foundation is dedicated to the renewal of primary care, to strengthening the ethos of its social mission, and to the sustainability of physician-owned private practice; a fundamental venue that is essential to the development of a high-performing healthcare system and to improving the continuity, access, and integrity of patient-centric care for individuals, communities, and populations.
Working on the premise that technology is a tool, and not a goal in itself, the foundation provides membership-based programs and services that enable primary care practices to effectively adopt new technologies, to develop capacity for innovation, and to foster a sustainable culture of quality improvement and continuous learning.
The foundation serves as an overarching community infrastructure for private practices; coordinating the development of support networks, knowledge sharing opportunities, research partnerships, entrepreneurial leadership, and policy development efforts.
The Foundation is a collective of public, private, and non-profit organizations who share common interests and work together in partnerships and collaborations to address the structural inequality of our healthcare system through multi-level, institutional change.
